In Which Diseases Is Total Hip Replacement Used?

Total hip replacement is primarily performed in middle age and later. There is no upper age limit for this surgery. Depending on the indication, it can be applied to anyone who has completed or is about to complete bone development. It is particularly a definitive solution for young adults aged 20-40 suffering from developmental hip dysplasia (hip dislocation) or avascular necrosis (a blood supply issue in the femoral head). The surgery provides excellent results, eliminating pain, limping, and leg length discrepancy.

In general, conditions requiring hip replacement include:

Osteoarthritis (joint degeneration)
Hip dislocation and childhood hip disorders such as slipped capital femoral epiphysis
Rheumatic diseases
Post-infection complications
Bone tumors
Hip fractures in older individuals
Bone necrosis due to blood supply issues
Patients with these conditions who do not benefit from or are unlikely to benefit from non-surgical treatments are candidates for hip replacement surgery.

How Is Hip Replacement Surgery Performed?

If the patient does not have an active infection (such as urinary tract, throat, or dental infection), blood tests are conducted, and the anesthesiologist is consulted for surgery preparation.

If no issues arise, the patient is admitted to the hospital on the day of surgery or one day prior.

Conditions like diabetes and hypertension are not obstacles to the surgery but must be well-controlled.
Smoking is discouraged as it increases the risk of infection.
The surgery can be performed under general anesthesia (full sedation) or spinal anesthesia (where the patient remains awake).
A 10-20 cm incision is made on the hip, depending on the surgeon’s preference.
Damaged bone structures are removed, and prosthetic components are implanted.
The capsule, muscles, fascia, subcutaneous tissues, and skin are then sutured.
After surgery, the patient is transferred to their hospital bed.
Oral feeding is allowed approximately 4-6 hours after surgery.
Patients are encouraged to walk with support on the evening of the surgery or the following day.
Compression stockings and a walker must be used during the early recovery period.
Wound dressings are changed every two days.
